full-time position
offers an exciting opportunity to join a team of dedicated health care professionals that pride themselves in providing quality resident care. Our facilities have a prominent reputation in the community for providing excellent rehabilitation services.
Baywood Court Health Center
, located in
Castro Valley, CA
, has 54 beds and is part of a tri-level community (Assisted Living, Independent Living and Skilled Nursing). An outpatient program allows our center to be a part of our client’s continuum of care.
Our strong rehab team is consistently rated high by our customers, via customer surveys. We have multiple therapists with neuro certifications (NDT), a therapist certified in ergonomics, an OT who is dysphagia certified, and two OT’s who are in the process for physical agent modalities certification. In addition, two of our PT/PTA’s are CI’s. Programs with increased training, but not limited to: neuro, vertigo, pain management with modalities, and dysphagia.
Our patient populations include: orthopedic, dementia, CVA, TBI, cardiac, pulmonary, and more. The therapy space includes sufficient equipment for skilled nursing modalities: hydrocollator, ultrasound, e-stim., and diathermy.
Located in Castro Valley, in the East Bay of California area, Baywood Court Health Center is approximately 30 minutes from San Francisco.
Put your physical therapy skills to work where they're really needed - evaluate a resident's condition, develop a treatment plan, and help them get better, day by day. You'll also instruct the nursing staff and the residents' families on follow-through programs that build on the progress they've made.
Responsibilities
Communicate resident progress or problems to supervisor and other team members; assist with resident scheduling and post charges daily to resident records.
Document resident care in accordance with Peoplefirst, regulatory, licensing, payer and accrediting requirements.
Instruct resident's family or nursing staff in follow-through programs.
Maintain equipment and work area in a safe and clean condition.
Make presentations to support marketing efforts, at team conferences and in-services.
Handle job responsibilities in accordance with the Company's Code of Business Conduct, the Corporate Compliance Agreement, appropriate professional standard and applicable state/federal laws.
Qualifications
Valid and current Physical Therapy license or equivalent, in the state(s) where services are rendered.
