Reporting to the Regional Leader, Continuity of Care, this position is responsible for providing leadership for home health & hospice agencies w/in the No or So CA Region, & for working collaboratively w/ the regional counterpart to monitor division-wide compliance w/ state & federal regulatory requirements related to home health/hospice. Ensures that consistent processes & program objectives extend across all home health & hospice agencies w/in a Region by collaborating w/ the Continuing Care Leaders in achieving a coherent, integrated whole. Develops region-wide & statewide systems & approaches for monitoring & managing the care of members receiving home health & hospice services. Develops strong collaborative relationships w/ Continuing Care Leaders, division directors of continuing care services & regulatory services (QMRS), & the Medical Groups to achieve regional & division goals. Leads efforts to ensure the rapid transfer of best practices.
Essential Functions:
- Provides leadership for home health & hospice services throughout the region & for the state, through collaboration & joint accountabilities w/ the Continuing Care Leaders
- Monitors home health & hospice compliance w/ utilization mgmt stds & accreditation & licensing requirements to include meeting all regulations & stds related to Medicare, JCAHO, Title 22, & other federal, state, & local req'mts
- Ensures that measures are developed & in place & appropriate goals are set to meet performance stds in the delivery of home health & hospice services
- Escalates quality & compliance concerns throughout the organization in a prompt & timely manner
- In collaboration w/ the PMGs & HP/H Divisional & SA leadership, creates the processes to implement quality & utilization stds for home health & hospice services to ensure coordinated plans of treatment, delivery systems that meet member needs, & cost effective utilization of necessary services
- Articulates issues or problems from a broader organizational/ mission perspective
- Champions improved ways of meeting targets or goals
- Anticipates & plans for future issues or problems
- Collaborates w/ the Continuing Care Leaders on the following joint accountabilities
- Compliance consistency & monitoring (note: oversight, auditing, & compliance expertise in conjunction w/ Region-wide & Statewide Business Line Direction & Strategy, Goal Setting, Performance Monitoring, Workforce Planning, Systems Development & Support, Database Development/Mgmt, Implementation of Best Practices & Service Outcomes & Consistency
- Meets regularly w/ the No & So Home Health & Hospice Service Dirs in their existing forums & as a member of their peer group
- Meets w/ the Continuing Care Leaders on a regular or as needed basis, driven by issues of mutual concern
- In collaboration w/ the Continuing Care Leaders monitors & plans for appropriate home health & hospice capacity & resources across the state to ensure adequate access to services
- Where capacity of resource constraints exist, works w/ regional & statewide leadership to identify options for providing care
- In collaboration w/ the SA Continuing Care leader & Home health/ Hospice agency Dir, assists agencies in the implementation of new practices or systems
- The Continuing Care Leaders remain locally accountable for the following: Direct Patient Care & daily operations, local quality & regulatory compliance, Medical group relationship mgmt, Budget, Member Satisfaction, Continuum of Care Mgmt, & Edu & Skills Training
Basic Qualifications:
- Ten (10) or more years (usually) of relevant and progressive experience in clinical and management roles in a integrated delivery system or delivery network with multiple facilities/agencies to include recent home health program management experience
- Master's degree in health services or business administration, public health, nursing or related discipline
- Current R.N. license required
- Working knowledge of quality assurance, resource management, health plan regulatory and licensing processes including Medicare and MediCal regulations related to home care and hospice
- Ability to work at both a strategic and operational level
- Demonstrated expertise in project management, verbal and written communication skills and interpersonal skills such as negotiating, influencing, and collaborating
- Must be able to work in a Labor/Management Partnership environment
