Current California Pharmacy License Required. Experience in unit dose distribution and IV admixture. Hospital experience preferred. Valid fire card obtained within 90 days of hire requir! ed. Must be available weekends.
Job description/Responsibilities
Monitors all posting onto the patient profiles in order that accuracy, drug allergies, drug interactions, duplicate therapy, rational drug therapy and incompatibilities may be realized and better patient care received. Checks patient medication cassettes prior to delivery to patient care areas to ascertain that the correct drug, dosage form, schedule and strength is being dispensed. When necessary, prepares IV admixtures and extemporaneous parenteral products under aseptic conditions. Answers questions and provides information to physicians, nurses, patients, and other healthcare professionals on the appropriate use of medications, drug interactions, side effects, dosage, storage, etc. Maintains established procedures concerning quality control, security of controlled substances and disposal of hazardous waste. Verifies orders as outlined in approved policies and procedures. Assists in monito! ring practices related to adverse drug events and/or medicatio! ns errors. Accurately compounds, labels and dispenses medications to patients. Assists in the coordination, planning, and supervising of the day-to-day department activities in accordance with established hospital policies and procedures, accepted standards of practice and Federal and State laws. Participates in Performance Improvement activities by collecting data. Monitors patient profile to assess drug therapy for appropriateness and safety consistent with a high level of patient care, including range of dosing, drug allergies, drug interactions, and polypharmacy. Reviews adverse drug reactions, communicates occurrence with physician and counsels patient when appropriate. Effectively communicates with physicians, nurses, either verbally with read back and/or in a clear precise written orders. Assures that each patient profile contains accurate and appropriate demographic data (weight, height, allergies/reactions, etc.) Serves as the pharmacy liaison between the departmen! t, medical staff, and other hospital personnel on matters pertaining to drug distribution, drug therapy, and pharmacy policies and procedures. Accepts responsibility for resolving immediate problems in pharmacy service, patient complaints, procedural issues among support staff, drug procurement and distribution, drug furnishing irregularities, emergencies, etc. Consistently demonstrates ability to keep up with patient workload. Completes all daily tasks within assigned shift. Maintains security of Pharmacy. Performs other duties as assigned or required.
