RN CASE MANAGER

EDUCATION REQUIREMENTS/PREFERENCES: Licensed Registered Nurse required Graduate of a state approved RN program BSN preferred. Certification: Advanced certification is preferred (CCM, AB! QAURP or ACM)
EXPERIENCE REQUIREMENTS/PREFERENCES: Minimum of three years in acute care hospital experience and one to two years case management experience required.
Communication: Excellent verbal and written skills required. Must read, speak and write English fluently. Bilingual skills in other languages preferred.
Specific Skill Elements: Excellent organizational skills required. Familiarity with the following: ANA “Nursing: Scope and Standards of Practice†ANA “Code of Ethics for Nurses with Interpretive Statements†Nurse Practice Act for state of employment
Technology: Basic understanding of desk top computers and work related software.
RN Case Manager will be responsible for coordinating patient care from pre-admission ,through hospitalization to post discharge follow up, in collaboration with the heath care team, to meet the patient's need. Responsible for the attainment of desired lengt! h of stay and continuum of care outcomes from the Case Managem! ent of all systems and services. This is done using a multidisciplinary team approach to care. Uses a problem solving approach that includes assessment, goal definition, plan development /monitoring, evaluation and revision of the plan to achieve optimum patient outcomes. Communicates plan to the Interdisciplinary team, including the physician, and patient and family.
