The VA Greater Los Angeles Healthcare System (VAGLAHS) is seeking an experienced Nurse Practitioner (NP) to work in our Home-Based Primary Care (HBPC) Department. The NP will provide home-based primary care to a panel of Veteran patients in a patient-centered, healthcare team approach. The NP will be responsible of provide direct and indirect patient care that includes assessment, diagnosis, planning, and treatment ! of health problems for Veterans in the Los Angeles area. The HBPC NP is a midlevel practitioner who provides primary care in the home to a clinically and psychosocially complex population of chronically ill or severely disabled veterans. Duties include assessment, planning, coordinated provision, and evaluation of individual treatment plans for patients receiving care from the Home Based Primary Care (HBPC) program. The NP is responsible for leading the interdisciplinary HBPC team in provision of care to their individual caseload of patients. He/she collaborates with a lead physician designee in the team assigned and is accountable for outcomes of his/her practice. The HBPC NP is accountable to the HBPC Program Director and reports directly to the Geriatrics and Rehabilitative Care (GRC) Lead NP. This position is not open to novice Nurse Practitioners.

Monday-Friday Day Shift

Duties include, but are not limited to:
1. Evaluates the veteranÂ's ph! ysical health, family and psychosocial history to identify health care ! needs and documents in medical record

2. Provides home visits and provides primary and continuity of care to patients and families in the home setting

3. Performs initial and interim history and physical examinations

4. Maintains expected work load panel of patients, assuring their continuity of care and that health maintenance and performance measures are achieved

5. Provides health education to Veterans

6. Uses CPRS to document patient physical exam, treatment and plan for care.

7. Completes all relevant clinical performance reminders and participates in quality improvement activities and peer review

8. Assists with the design and implementation of delivery systems/ programs to facilitate the care of high risk veterans

9. Collaborates with team members to provide continuity of clinical care and implement of treatment plan

10. O rders laboratory, radiologic and other diagnostic tests and provi! de initial interpretations

11. Prescribes medications in accordance with scope of practice

12. A ssesses patient learning needs, development, and implementation of plan to meet needs, and instructing patients and families in self-care and treatment

13. Participates in medical and/or nursing research and quality improvement

14. Screens patients for disease prevention and identifying health risks

15. Formulates a diagnosis and development of treatment plan and reviews patient management with physician as appropriate QUALIFICATIONS REQUIRED:

BASIC REQUIREMENTS:
U.S. citizenship. Non-citizens may be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.

Full unrestricted Licensure, Certification, or Registration required

Graduate of a school of professional nursing approved by the appropriate State-accrediting agency and accredited by one of the following ac! crediting bodies at the time the program was completed by the applicant! : The National League for Nursing Accrediting Commission (NLNAC) or The Commission on Collegiate Nursing Education (CCNE). In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing. The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. Students should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript. (Reference VA Handbook 5005, Appendix G6)

Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia.

EDUCATION/EXPERIENCE REQUIREMEN! TS:

  • A MasterÂ's Degree in Nursing (MSN) or Doctorate of Nursing (DNP)
  • Basic Life Support (BLS) certification from the American Heart Association, American Red Cross, or Military Training Network
Preferred Experience:
  • National Certification as a Nurse Practitioner
  • Current Furnishing License
  • Current DEA certificate for controlled substances schedules II-V
  • Computer literacy proficiency with in-dept experience with the use of Microsoft Office programs or comparable word processing, spreadsheet and graphic software and the ability to learn new programs specific to the VA.
  • Minimum (3) three years clinical registered nursing practice, preferably experience in the care of geriatrics and/or medical care for patients with multiple co-morbidities.
ExpExperience in managing patients in a home environment is preferred.

  • Minimum (1) one year previous NP experience in a! n Ambulatory or Primary Care setting
  • Knowledge and experience ! with comprehensive Diabetes Management
  • Must have a valid Driver's License as this position requires driving and seeing Veterans in the community
GRADE DETERMINATIONS:
Experience as a RN will be evaluated to determine if the experience is of an acceptable level of quality with regard to the following four dimensions of nursing: Practice, Professional Development, Collaboration and Scientific Inquiry. Each dimension of practice has criteria that demonstrate essential core competencies for VHA Nursing Personnel.

The criteria are as follows:
Practice: practice, ethics and resource utilization;

Professional Development: performance, education/career development;

Collaboration; collaboration, collegiality;

Scientific Inquiry: quality of care, research.

PHYSICAL REQUIREMENTS: This position requires visual acuity, keen hearing, c! lear distinctive speech, and manual dexterity. This position requires p! otentially long periods of continued walking, standing, stooping, sitting, bending, kneeling, pulling, and pushing. Transferring patients and objects may be required. The candidate may be exposed to infected patients and contaminated materials and may be required to don protective clothing in isolation situations or operative/invasive procedures. The candidate may occasionally be exposed to patients who are combative secondary to delirium, dementia, or psychiatric disorders. The incumbent must be a mature, flexible, sensible individual capable of working effectively in stressful situations, able to shift priorities based on patient needs. The candidate will be working closely with others and may have protracted or irregular hours of work.
