The AIM implementation is a three-year Health Care Innovation Award funded by the Center for Medicare & Medicaid Innovatio! n. AIM is a patient-centered and evidence-based model of care ! for patients with advanced illness that is specifically designed to address the gaps in our current health care system that these patients routinely experience. Through this project, Sutter hopes to clearly show that AIM is a highly effective, replicable, scalable model that can be implemented nationally.
AIM provides nurse-led care management, palliative care, and advance care planning for patients with late-stage chronic illness. AIM is provided at no cost to patients and their families.
The expansion of AIM system-wide will provide additional services to coordinate care across the network for patients with advancing chronic illness.
Under the supervision of the Clinical/Manager, with guidance from the AIM Program Manager and AIM Team Leader, the AIM HH RN Care Coordinator (RNCC) provides specialized nursing services to patients nearing the end of life. The AIM HH RN Care Coordinator (RNCC) manages a caseload of patients with advanced illness an! d functions as a consultant to other Homecare staff regarding appropriateness of patients for the program, symptom management and end of life issues. The care focuses on symptom management, advanced care planning, education relating to disease process and treatment options, setting goals of care with patient and family, reduction of Emergency Room (ER) visits, inpatient hospitalizations and timely transition to hospice. Depending on the branch census, the AIM Nurse may also case manage regular homecare patients. This position works closely with the Hospice staff to ensure high quality symptom management. Other duties associated with the AIM program will be assigned as needed.
Qualifications:Education:
RN graduate from an accredited curriculum. Bachelor's degree in nursing or health related field preferred.
Licensure:
Current California RN license; valid California driver's license; current auto insurance; current BCLS/CPR certi! fication.
Exp erience:
- One year of acute m! edical experience strongly preferred. One year of experience in a home health and/or hospice position preferred. Oncology, Critical Care, Hospice, Heart Failure or palliative care experience preferred
- Completed Sutter Care at Home orientation and demonstrated proficiency
- No pending personnel actions
- Charting, productivity, compliance are at expected target levels
- Good clinical, critical thinking and assessment skills
- Good interpersonal skills
- Desire to work with patients with advanced illness
- Recommendation of supervisor/manager
Understanding of home care and hospice services and regulations, Title XXII, Medicare Conditions of Participation, JCAHO home health and/or hospice accreditation requirements, QI outcomes/benchmarking, infection control; good understanding of reimbursement patterns for Medicare and other payors; good working understanding of budget and other financial rep! orts.
