Title: CareSupport Community Coordinator

The SFHP CareSupport program is focused on managing care for high risk members with heavy use of acute health services including h! ospitals and emergency departments. The current interdisciplin! ary team consists of one maste r ’ s level social work supervisor , one master’s level MFT supervisor, and bachelors level Community Coordinators with input from physicians.
The Care Support Community Coordinator (CSC Coordinator) will be responsible for helping Health Plan members n avigate the healthcare system and get the best care in the right setting. The CSC Coordinator will work as an integral part of the interdisciplinary CareSupport team with the aim of improving health outcomes , coordinating care across the San Francisco health and social services community, and enhancing the use of primary care and other outpatient supportive services for vulnerable patients with complex health needs.
The C S C Coordinator will conduct in person biopsychosocial assessments , create person- focused holistic care plans, and will function as a liaison, coordinating services among multiple providers and community agencies. T his position will be based within the SFHP off! ice but will involve substantial time in the community (at clinics and hospitals , with community-based organizations, and home visits) .The CSC Coordinator is expected to function independently in the field using mobile technology for regular communication with the team and should be comfortable with online clinical documentation.
ESSENTIAL DUTIES & RESPONSIBILITIES

  • Conduct initial telephone or in-person assessments for patients referred for care coordination services or case management, assessing medical history, psychosocial issues, and level of function
  • Participate in the development of a member-centered, individualized Care Plan . Support the CareSupport team with implementation of patients’ care plans:
Ø Support patients in identification of their own strengths and barriers to help them be successful with their care plans
Ø Assist member in navigating the system of providers and social service agencies
Ø Support patien! ts and their caregivers in re-assessment and modification of Care Plan ! goals
Ø Help connect patients to cultural, community and social resources

  • Actively participate as a member of the SFHP CareSupoort team, providing input and creative strategies to address health issues and promote wellness
  • Support and educate individuals in medication choice, management, and adherence; exercise; nutrition; health care system navigation, substance use and other health topics
  • Assist patients or providers who call with questions about care coordination and management.
  • Complete all required documentation in a timely fashion in accordance with regulations and office standards
  • Identify opportunities for prevention and screening through interviews and disease registries, and work with nurses and social workers to ensure patients get the right care at the right time
  • Interface with SFHP Health Improvement and Utilization Management team in co-managing members
  • Act as a liaison to hospital s ! , long-term care settings , outpatient providers, home health representatives , and community agencies
  • Participate in regularly scheduled staff development trainings . Attend and participate in regularly scheduled supervision meetings and be prepared to share both successes and challenges of working with individuals and the CareSupport team
  • Communicate all concerns to health care team and Supervisor as necessary seeking support and guidance proactively
  • Represent the program to internal and external customers
  • Other duties as assigned.

CORE COMPETENCIES / KNOWLEDGE & SKILL REQUIREMENTS
  • Knowledge of San Francisco County community resources , Medicare, Medi-Cal, medical codes is helpful but not required
  • Basic medical knowledge including the ability to recognize social and medical risk factors, knowledge of trauma symptoms and behaviors, and chronic disease conditions
  • Case Management experience ! a plus
  • Ability to recognize mental illness and addiction issue! s
  • Ability to work collaboratively with individuals, listening to, implementing, and advocating their treatment suggestions
  • Working knowledge and use of Trauma Informed language and principles a plus
  • Working knowledge and use of Harm Reduction language and principles a plus
  • Working knowledge and use of Person Centered language and principles a plus
  • Ability to display non- judgmental attitude
  • Ability to collaboratively work on realistic goals with individuals on their way to recovery
  • Attention to detail is vital as is maintaining focus and meeting deadlines
  • Ability to motivate others
  • Ability to engage and work collaboratively with others, including patients, patient families, clinical team members and community resources
  • Demonstrated self motivation and the ability to work independently
  • Ability to work with people from diverse backgrounds and experiences
  • Ability to open! ly address and acknowledge issues of substance use and mental illness
  • Cultural knowledge and sensitivity to the population being served
  • Basic word processing and computer skills
  • Excellent English written and verbal skills
  • Oral and written proficiency in Spanish or Cantonese is a plus
  • Proficient use of common Microsoft Word applications such as Word, Excel, Outlook and Access
EDUCATION, EXPERIENCE & TRAINING REQUIRED
  • Minimum one year experience in community based outreach
  • Bi-cultural, bi-lingual language skills preferred
  • Associates or Bachelor’s Degree . Preference may be given to those with a BSW (Bachelor’s Degree in Social Work)
  • Experience working with people with mental illness, addiction issues , and/or homelessness
  • Experience with Medi-Cal and/or Department of Health Care Services regulations and standards preferred
  • Knowledge of and experience with stat! e and federal insurance programs a plus
