More details about this occupation opportunity please read the description below. Seasons Hospice & Palliative Care is seeking a Clinical Liaison to join our team of professionals. We make a difference every day to patients and families coping with end of life illness in creating a center of excellence in hospice care.
Clinical Liaison, R.N., acts as liaison between the contracted hospitals and the hospice team, makes inpatient visits to hospice patients in contracted facilities, acts as consultant for pain and symptom management, provides in-service education to providers, performs pre-admission evaluations for appropriateness for hospice services, initiates the admission process by educating and informing patient/family of hospice and its services. Also obtains informed consents. Provides nursing assessment, initial plan of care and intervention(s), which optimize patient and family directed comfort and quality of life, and coordinates discharge planning. *Please note that all clinical decisions must be made under the direction and supervision of the Clinical Director and in accordance with regulation admission/eligibility by the Medical Director.
RESPONSIBILITIES:
1. Provides community and provider in-service education in assigned territory.
2. Maintains daily communication with Admissions Department and Team Directors.
3. Works with the Hospice Care Consultants to develop and implement individualized marketing plan.
4. Maintains visibility and accountability to increase relationships with contracted inpatient facilities and physician groups.
5. Acts as a liaison with Seasons Hospice Medical Director, Team Physicians and Hospice Physicians on behalf of providers and patients/families.
6. Makes inpatient visits at contracted facilities and communicates with Admissions Department, Team Directors and/or Primary Nurse.
7. Provides consultation on hospice and palliative care.
8. Demonstrates familiarity with policies of the agency and state(s) and federal regulations.
9. Participates on committees and special projects and other related duties as assigned.
10. Performs evaluations, sign ups, assessments, and discharge planning of new patients primarily in contracted hospitals.
11. Formulates the initial interdisciplinary plan of care and communicates to the Team Director.
12. Obtains data on physical, psychological, social and spiritual factors that may influence patient/family health status and incorporate that data into the plan of care.
13. Turns in all paperwork pertaining to sign ups and assessments within 24 hours to Clinical Services.
14. Maintains confidentiality of sensitive information.
15. Participates in hospice orientation and ongoing education programs.
16. Attends outside seminars or educational opportunities to increase knowledge and skills.
17. All other duties as assigned.
QUALIFICATIONS:
1. Registered nurse currently licensed in the state(s).
2. Board Certification or working toward board certification in hospice and palliative care.
3. Demonstrated commitment to Seasons Hospiceâs philosophy of care, values, mission statement and organizational culture
4. Previous experience in hospice, home health, geriatrics or general medical surgical nursing.
5. Willingness to work a flex schedule and serve patients/families in the geographic service area.
6. Detail oriented with good time management skills.
7. Above average documentation skills.
8. Management/leadership skills.
