Pathways Home Health & Hospice is celebrating over 35 years of service in the Greater Bay Area and has an immediate ! opening. We currently have an excellent opportunity for a Director of Admissions. AREAS OF RESPONSIBILITY : The Director of Admissions establishes and maintains a work environment that assures high quality, cost effective and efficient patient referral processes. The ideal candidate develops and coaches staff to meet established performance expectations. They assure the Referral and Information Team is knowledgeable and complies with regulatory and accreditation requirements as well as agency administrative and clinical policies and procedures. This individual assists the Intake and Liaison nurses in processing referrals and requests for information. He or she assures timely information/evaluation visits, oversees Health Plans and provides consultation to the durable medical equipment (DME) functions for Home Health and Hospice patients. This individual also performs other duties as assigned consistent with skills and training for this position in alignment with the goals a! nd mission of Pathways Home Health & Hospice. QUALIFICATIONS: !
Registered nurse, licensed to practice in the State of California; BSN required.
Minimum of 2 years of home health, hospice or related health care experience within the past 5 years.
Supervisory experience in a health care setting.
Strong clinical nursing background.
Knowledge of Hospice philosophy, Home Health and Hospice services, requirements and governing regulations, accreditation standards and payor coverage.
Excellent communication and customer service skills.
Good telephone interviewing and assessment skills.
Strong organizational skills with the ability to work independently.
Must have ability and willingness to cross-train into neighboring functions.
Must have the ability to work effectively in a team environment.
Current California Driver’s license; valid automobile insurance and a personal automobile available to use for work.
Able to maintain a flexible work schedule as well as overseeing weekend and holiday ref! erral functions.
