The Transplant Coordinator/Nurse Practitioner responsibilities include all phases of the transplant process from initial referral through outpa! tient follow up. This position will be responsible for providing comprehensive assessment, diagnosis, planning, implementation, and evaluation to pediatric heart transplant candidates and recipients. The responsibilities include:
Coordination of an interdisciplinary medical care delivery system under the direction of the Medical Director
Serving as a liaison between patients, family, medical staff, referring physicians, and insurance companies
Developing and providing individualized patient/family teaching
Provide ongoing assessment of appropriate nursing interventions for effective use of resources and quality assurance. Additional responsibilities include rotational on call duties for donor/recipient heart lung transplant coordination and liaison function with organ procurement agencies, data collection and documentation for clinical management and certification requirements, developing educational/teaching materials and protocols; development of clin! ical protocols, policy and procedures; community education and! public relations.
Skills / Requirements
MINIMUM QUALIFICATIONS:
Master's Degree in Nursing - Graduated from an accredited Nurse Practitioner program (preferred Acute Care Program)
Pediatric Critical Care experience (preferably in cardiac specialty area)
Pediatric Cardiac Transplant experience
Pediatric NATCO certification (in process or within 6 months of hire)
Nurse Practitioner, State of California
Registered Nurse, State of California
CPR certification (Issued by American Heart Association)
PALS certification
Nurse Practitioner Furnishing, State of CA (for positions that require medication furnishing/ordering/prescribing) upon hire but obtain no later than 12 months of employment or sooner based on department needs- See Job offer letter
Nurse Practitioner Furnishing, State of CA, Controlled Substance II Privileges (for positions that require the writing of sched! uled II medications) upon hire but no later than 1 month after obtaining NPF
DEA number (for positions that require the writing of scheduled medications - Each department to identify which class needed CS II thru V) upon hire but no later than within 1 month of obtaining NPF & CSII privileges and no later than 12 months of employment whichever comes first
Medicare Billing number (NPI- National Provider Identifier) upon hire but no later than 3 months of employment
National Certification in specialty area as a Nurse Practitioner (eligible & complete within 6 months)
PREFERRED QUALIFICATIONS:
2 years of experience
Controlled Substance II privileges
National Certification in specialty area as a Nurse Practitioner
2 years of NP experience with emphasis in specialty area
Experience in specialty areas as a Nurse Practitioner
Formal education and/or lecturing skills
Preferred Pedia! tric NATCO certification and Cardiac Transplant experience
