Qualifications
- Bachelor of Science in Nursing required
- Mast! er's degree preferred
- Current unencumbered California Registered Nurse required
- Basic Cardiac Life Support (BCLS) required
- 5 years current clinical experience in critical care, progressive care, intermediate care, ambulatory care, cardiac rehab or cardiology specialty
- Advanced clinical skills required
- Experience in the area of CAD, CHF management, diabetes, hypertension, hyperlipidemia, COPD, or Asthma disease management required
- Broad knowledge of medication class actions and adverse effects required
- Bilingual (English/Spanish) preferred
The Population Health Department at SRS is expanding. This position is for an expert clinical generalist with relevant experience caring for individuals with diabetes and possible co-morbid CAD, CHF, COPD, and asthma. The RN Case Manager will assess populations with diabetes and lipids and blood pressures above the target range. The RN will outreach to in! dividuals and provide education, coaching, and clinical evalua! tion and follow up to promote optimum adherence to the plan of care and improvements in clinical outcomes for this population at high risk for heart disease and its many complications. The RN Case Manager will communicate with the primary care providers and make appropriate recommendations to the plan of care based on telephonic patient assessments and advanced clinical knowledge in the area of lipid management. The RN Case Manager will outreach to a large population of individuals and demonstrate advanced organizational skills to provide timely contact, laboratory test opportunities, follow up and evaluation. The RN Case manager will use advanced communication skills and motivational interviewing techniques to identify barriers to care and develop solutions with the patient preferences a priority. The RN Case manager will be responsible for the outcomes of the population as reflected in the mission of the organization. She/he adheres to established policies and performs ri! gorous documentation according to established standards. Performance and patient outcomes are monitored at regular intervals. This Case Manager position is with the Population Health Department of Sharp Rees Stealy (SRS). The Population Health Department includes the Ambulatory, Disease Management and Diabetes Management programs. The department employs case managers who telephonically manage SRS patients with cardiac disease, diabetes, COPD or asthma whose current status is below optimum. The team includes, but is not limited to Case Managers, Diabetes Specialist Educators and support staff.
