Hours:
8 hour shift: I! ncluding weekends. Shifts can vary ie: days, pms, or nights as availability dictates.
Qualifications:
Current California Licensed Vocational Nurse licensure required
Current BLS certification required
1 year hospice experience required, acute care experience preferred
Knowledge of nursing process required
Effective interpersonal and communication skills required
Strong intermediate computer skills required
Good time management skills
Ability to perform all baseline clinical nursing interventions
Strong electronic documentation skills a must
Fluent knowledge of English, both verbal and written
Summary:
This position provides 'shift crisis care' to patients in their homes. You would be managing patient's symptoms related to the terminal diagnosis and medication administration as applicable. Will be working closely with patients and families at end of life. !
The LVN will work as part of a team led by the RN. If! the patient requires RN intervention the answering of patient calls will turn over to the LVN while RN is in the field. Additional job duties will be assigned in support of Access Team.
Sharp Hospice Care is dedicated to providing comprehensive care and compassionate support to individuals facing life-limiting illnesses, along with their families and friends. Services range from relief of physical discomfort to emotional and spiritual support. Sharp Hospice Care also provides “pre-hospice†home based palliative care consultation for patients with advanced illness such as heart failure, chronic obstructive pulmonary disease and dementia.
Sharp Hospice Care provides in-home services and also provides the region’s first freestanding hospice homes, LakeView Home in La Mesa and ParkView Home in Del Cerro, which offer end-of-life patients an alternative to nursing home placement. The homes â€" staffed 24 hours a day by licensed nurses â€"provide a comfort! able and supportive environment for up to four patients at one time in each home.
