As an NP/PA, you will be enlisted to provide comprehensive care in a primary care setting to La Clinica patients. Your core responsibilities will include providing medical triage of patien! ts, medical care management, and patient education and counsel! ing to a panel of patients. You will serve as a member of the cross-functional team of providers, engaged in routine and preventive primary care, care for chronic illnesses, urgent care, and telephone triage. You will be expected to makes suggestions for improvements in clinic flow, patient scheduling and service delivery, and collaborate professionally with interdisciplinary team members, including providers, health educators, medical assistants, registered nurses, and case managers.
The role is most suited to a PA or NP with 1-2 years of work experience in a primary care clinic setting, and with previous experience providing comprehensive outpatient medical care. Additionally, the individual in this position will make referrals to specialty providers and follow-up to ensure that patients’ bio-psychosocial needs are addressed. The NP/PA will consult with physicians on the management, assessment and treatment of specific cases as needed, and per protocol. This ro! le requires flexibility in meeting patients’ needs, and calls for an individual who is able to work evenings and Saturday triage clinics, and conduct in-home assessment of patients as appropriate. Overall success in this position will require top-notch time management skills, a high degree of interpersonal competence with both staff and patients, and thorough knowledge of evidence based approaches to medical care. Required Skills
Nurse Practitioner: Requires a current license from the California Board of Registered Nursing, a certificate of completion of a Nurse Practitioner Program.
Physician Assistant: Requires a current license from the California Physician Assistant Committee, graduation from an approved program of instruction in primary ! health care as attested by the American Medical Association, having pas! sed a certification examination administered by the Bureau of Medical Quality Assurance..
Cultural competence working in diverse/low income communities.
Bilingual English/Spanish preferred
Required Experience
One to two years of NP/PA experience working in primary care clinic setting preferred.
