Functions independently as an advanced nursing professional in palliative care across the health care continuum under direction! of the palliative medicine physicians and administrative guidance. Assesses the patient to identify palliative care needs; creates and modifies a plan of care; makes appropriate referrals; provides patient and family education; monitors patient status; and evaluates the effectiveness of the established care plan. Serves as an extension of the palliative medicine physician and a resource for medical and nursing staff and works collaboratively to develop and implement palliative care program in across all hospital functions. Demonstrates a high degree of clinical expertise in providing care for complex palliative care patients and their families.

Minimum Position Qualifications:

• Education: Master’s Degree in Nursing required.
• Experience: At least three (3) years recent experience in acute hospital setting.
• License / Certification: Current California RN licensure; current California Nurse Practitioner licensure; certification a! s a Nurse Practitioner by the State of California; current BLS! .

Preferred Position Qualifications:

• National certification within specialty (Advanced Practice Hospice or Palliative Nurse (ACHPN)
• Current additional certification as discussed with Manager (e.g. CHPN, National Pain Certification, etc)
