Exam Number J1418A Filing Type Open Continuous Filing Start Date 07/19/2013 Salary Type Monthly Salary Minimum $3,967.45 Salary Maximum $5,203.27 Position/Program Information Positions allocable to this advanced journey/expert level class are under limited supervision of a higher level supervisor; Health Information Senior Technicians perform, and may provide leadership or limited supervision over the provision of, highly complex services in the collection, management, coding and abstraction of health information. Essential Job Functions Reviews patients' health information for completeness, consistency and compliance with laws and regulations, including the Health Insurance Portability and Accountability Act (HIPAA), department and facility procedures and requirements, and Joint Commission on Accreditation of Healthcare Organizations (JCAHO) standards.

Leads and supervises a team of health information personnel and/or clerical staff engaged in reviewing pati! ents' medical files for completeness, consistency and complian! ce with Federal and State laws, Title 22 standards and departmental regulations as needed; provides direction and guidance to Health Information and medical records staff, providing basic and intermediate coding and abstracting services to the work unit.

Assures that all relevant medical documentation is included in patients' medical records, and tactfully and persistently seeks clarification, correction or completion of medical records or charts from responsible physicians, nurses, laboratories and other clinical staff; confirms pathologies, dates and times of treatments and procedures, and abstracts patients' medical records.

Abstracts and codes the most difficult inpatient cases, such as complex invasive surgical procedures, difficult disease categories and/or trauma, and cases involving multiple body systems and co-morbidities; facilitates patient discharge reviews and recommends additional coding.

Accurately selects principal diagnosis, seque! nce diagnoses and procedures and demonstrates significant knowledge of co-morbidities and other factors affecting the assignment of correct diagnosis related groups (DRGs) to patients' records.

Enters coded medical data into a computer terminal, and selects diagnoses and procedure codes from designated computer-based coding systems, such as 3M encoder; coordinates efforts with billing staff to maximize reimbursement, and resolves coding issues, such as unacceptable codes.

Expertly utilizes International Classification of Diseases, 10th Ed. (ICD-10-CM) and Health Care Common Procedural Coding System/Current Procedural Terminology (CPT-4/HCPCS) nomenclature, Ambulatory Payment Classifications (APCs), Diagnostic and Statistical Manual of Mental Disorders, Version IV (DSM-IV), and other coding and classification systems to maximize reimbursements and assure conformity with Title 22 and departmental standards.

Abstracts cancer patients' demographic and! morphology data, diagnosis(es) and treatments, provider data, diagnost! ic findings and treatment results and histology, tumor extent and behaviors (e.g., TMM staging), and related information as appropriate, and performs cancer registry quality checks on abstracts.

Trains and assists other health information management staff with patient records, coding and reimbursement issues and concerns; trains staff in new coding systems such as ICD, 10th edition, and coding and billing situations and factors unique to a specialty area, as required.

Assures the integrity of Medi-Cal billings and applies fraud, waste and abuse regulations and procedural controls to the work; may track and prepare control lists, conduct medical record quality reviews, and audit patient health information.

Verifies that cases of communicable diseases have been properly reported. Requirements SELECTION REQUIREMENTS:

OPTION I: Certification* as a Registered Health Information Technician (RHIT) by the American Health Information Manage! ment Association (AHIMA), -AND- two years of paid full-time experience in medical information management, coding and abstracting of medical records.

OPTION II: Certification* as a Registered Health Information Administrator (RHIA) by AHIMA, - AND- one year of paid full-time experience in medical information management, coding and abstracting of medical records. Physical Class Physical Class II - Light: Light physical effort which may include occasional light lifting to a 10 pound limit, and some bending, stooping or squatting. Considerable walking may be involved. License(s) Required A valid California Class C Driver License or the ability to utilize an alternative method of transportation when needed to carry out job-related essential functions. Certificate(s) Required *To qualify, applicants must attach or submit a copy of their certification as a RHIT or RHIA by AHIMA AT THE TIME OF FILING.

The required certification MUST be current and unrestric! ted; a conditional, provisional, probationary or restricted certificati! on will NOT be accepted.

Desirable Qualifications Credit will be given to applicants who possess DESIRABLE qualifications and indicate so on their employment application at the time of filing.

A Bachelor's Degree** in Medical Records Administration or a related field from an accredited*** college or University. Special Requirement Information
**In order to receive credit for the required degree, you must include a legible copy of the official diploma, official transcripts, or official letter from the accredited institution which shows the area of specialization with your application AT THE TIME OF FILING.

A mandatory tour of a Correctional facility and the Medical Services work environment will be required as part of the selection process prior to beginning the background investigation process.
Accreditation Information ***Accreditation: Accredited institutions are those listed in the publications of regional, national or internatio! nal accrediting agencies which are accepted by the Department of Human Resources. Publications such as American Universities and Colleges and International Handbook of Universities are acceptable references. Also acceptable, if appropriate, are degrees that have been evaluated and deemed to be equivalent to degrees from United States accredited institutions by an academic credential evaluation agency recognized by The National Association of Credential Evaluation Services or the Association of International Credential Evaluators, Inc. (AICE). Examination Content This examination will consist of an evaluation of training and experience based upon application information and desirable qualifications weighted 100%.

CANDIDATES MUST ACHIEVE A PASSING SCORE OF 70% OR HIGHER ON THE EXAMINATION IN ORDER TO BE PLACED ON THE ELIGIBLE REGISTER. Special Information

Eligibility Information The names of candidates receiving a passing grade in the examination will be placed on the eligible register in the order of their score group for a period of twelve (12) months following the date of promulgation.
