:

The

Case Manager

assists the organization by assuring that the right care is provided at the right place, at the right time. To accomplish these goals, the Case Manager: uses established criteria to evaluate the appropriateness of admission, level of care, and readiness for discharge; assures the timely movement of patients throughout the continuum of care by conducting concurrent reviews and resolving delays as necessary; assesses patients' post-discharge needs and develops and implements the plan; reviews physician documentation for accuracy and thoroughness and seeks clarification as necessary to improve clinical documentation; provides third-party payers with clinical information to assure reimbursement; and coordinates care with the treatment team , family and others as necessary. This position will be cross trained to the Sutter Care Coordination Program/Office Based Case Manager role and will be able to provide coverage in the physician office including Telemanagement for the AIM program.

Qualifications

MINIMUM POSITION REQUIREMENTS:
Education:

  • Current California RN Licensure. BSN strongly preferred
  • Most successfully complete Sutter Health Sacramento Sierra Region's case management orientation program and DRG Assurance Training within ninety days from date of hire.
  • National certification in Case Management (ACM, CCM) preferred.
  • Supervisory training helpful.
  • Must successfully complete Sutter Care Coordination case management orientation program within 90 days from date of hire.
  • Experience with in-home and telephone patient case management, knowledge of health-related information, education, resource and referral services
Experience:
  • Four years' previous acute care experience is required.
  • Previous case management experience is preferred.
  • Broad acute clinical background (experience in more than one population
and

level of care) strongly preferred.

  • Experience using InterQual is strongly preferred.
Knowledge/Abilities:
  • Ability to use clinical knowledge to identify potential quality of care issues, delays in service, and post-acute care needs required.
  • Basic knowledge of governmental (Medicare, Medi-Cal, etc.) healthcare reimbursement systems is required.
  • Advanced knowledge of healthcare reimbursement systems: HMO, PPO, capitated agreements, PPS, etc., is preferred.
  • Basic knowledge of post-acute healthcare resources such as types: SNF, Home Health, Hospice, etc., and their admission criteria, etc., is required.
  • Intermediate level computer skills are required.
  • General typing skills (35 WPM) required.
  • General knowledge of coding and DRG assignment process is preferred.
Special Skills/Equipment:
  • Must be able to effectively communicate with, and promote cooperation between, multiple people including: patients and their families; physicians, nurses, social workers, etc.
  • Able to orchestrate successful, complex discharge plans.
  • Must be able to develop an organized work plan in a high-volume environment with rapidly changing priorities.
  • Leadership skills to delegate, functionally supervise, provide direction/guidance to staff and hold others accountable are required.
  • Must have the abilities to work independently with a minimum of direction, anticipate and organize workflow, prioritize and follow through on responsibilities.
  • Must have strong clinical assessment and critical thinking skills necessary to provide utilization review/discharge planning services appropriate to patients with complex medical, emotional and social needs.
