Job Summary: Under general direction, provides a leadership role in formulating policy and instituting models that promote medication safety in all aspects of the medication management process. In conjunction with the Director of Pharmacy, the role is responsible for coordinating all activities related to medication management and medication safety provided by the facility pharmacy department. These activities involve reporting, system and process evaluation, formulation of recommendations and initiatives, coordination of intervention strategies, and monitoring of activities related to medication safety. This individual serves as the Medication Safety Officer and as a liaison to facility pharmacy, nursing, and the medical leadership. Qualifications: Education: Bachelor of Science degree in pharmacy. Doctor of Pharmacy degree, or approved American Society of Health-System Pharmacist residency preferred. Responsible for self-continuing education by attending conferences, meetings and seminars. Minimum Experience: Five to seven years of recent hospital pharmacy experience preferred. Three to five years of which are at a management/supervisory capacity. Required Licenses/Certifications: Possession of a valid pharmacy license to practice in California.

- Knowledge of Clinical Pharmacy and modern pharmacology.
- New developments in Hospital Pharmacy, such as Unit Dose and I.V. Additive; compounding, dispensing and storage of drugs and chemical supplies.
- State and Federal Laws governing use and dispensing of controlled substances, poisons and drugs.
- Skill in supervising and coordinating the pharmacy services in a large hospital setting.
- Advising and instructing others in pharmacy procedures and regulations, and on the properties and effects of drugs. Evaluating staffing, equipment and facility requirements; keeping records and preparing technical reports.
1. Maintains high standards within the department and translates pharmacy policies and procedures into the daily operation of the pharmacy in accordance with The Joint Commission (TJC) and the department of public health regulations. 2. Performs the duties of Director of pharmacy services in the event of his or her absence. Maintains, enhances and evaluates the pharmacy computer systems (mainframe and micro systems) for new/improved clinical services, including but not limited to the ADR and DUE programs, QA documentation and reports by ongoing revision of existing software files and development of new files. 3. Coordinates QA, ADR and DUE programs. Includes staff development programs for training new employees and continuous retraining of existing staff. 4. Assists in the development and implementation of process changes to improve medication safety; identify best practices for medication safety; analyzes current practices that contribute to medication errors and take proactive steps for prevention. Facilitate process and system changes and reduce the likelihood of occurrence and recurrence of error; analyzes medication occurrence reports on a monthly basis. 5. Serves as team leader for the Medication Safety team, a subcommittee of the P & T Committee and provides updates to the P&T committee; when requested, serves as an interdisciplinary member to the root cause analysis (RCA) team for sentinel events 6. Fiscal responsibilities include cost savings associated with improved clinical pharmacy services. 7. Communicates new policies, procedures and regulatory hospital matters to the staff and provides for the safety of the staff. 8. Coordinates medication deliveries in conjunction with coordinator of support services for the department. Maintains an orderly workflow for all medication dispensing activities in the Hospital. Ensures timely deliveries of all medications. 9. Coordinates the duties and assigns daily work assignments of all pharmacists; orients and trains new pharmacy personnel including interns and co-op students in dispensing activities as per hospital policies and procedures 10. Defines performance objectives and metrics for the department and assesses the level -of competence of staff in a timely manner; Promotes effective working relationships and works effectively as part of a department/unit team inter and intra departmentally to facilitate the department's/unit's ability to meet its goals and objectives; provides leadership and guidance to other leaders and staff in goal-setting, problem solving, resource management, and outcome achievement. 11. New program/project development: Determines feasibility of new services and implements changes in programs/services. 12. Responsible for inventory management and for maintaining a fixed level of medication and supplies; purchasing functions of the department. This includes review and analysis of all purchasing functions and documentation; responsible for review of charts for billing discrepancies noted upon third party audit.
