The VA Greater Los Angeles Healthcare System (VAGLAHS) is seeking an experienced Nurse Practitioner to work in our Community Care program. This program focuses on a Veteran-centered holistic approach to care starting with outreach on the streets, and continues to care whether or not the Veteran clients are prepared to accept mental health, substance abuse services and primary care medical services- and despite potential treatment failures and setbacks. The Assertive Community Treatment (ACT) Team consists of
psychiatrists
, social workers, community care workers and the APRNs. This team has a Â"street medicineÂ" approach which incorporates a trans-disciplinary care team model into the community and engages homeless Veterans. This NP will be providing primary care to Veterans! on the street or in shelters with potential follow-up care in! the clinic. The NP will collaborate with a lead physician designee in a team assignment. He/she is administratively responsible to the Associate Chief Nurse, Nursing Service Community/Primary & Ambulatory Care and will report directly to the Community Care Nurse Manager. This position is not open to novice Nurse Practitioners.
Job site location may be changed to LAACC, West LA or Sepulveda campuses to meet the needs of the organization

KEY REQUIREMENTS
U.S. Citizenship
Full unrestricted Licensure, Certification, or Registration required.
Must pass pre-employment physical examination.
Designated and/or Random Drug Testing may be required.

DUTIES:
Duties include, but are not limited to:
1.
Participates in outreach activities that find or identify homeless Veterans
2.
Develops innovative care strategies with the Outreach staff that serve to reintegrate the homeless Veteran population
3.
Assists with the design and implementation of delivery systems or programs that facilitate the care of high risk veterans
4.
Provides primary care with skill and sensitivity to Veterans who are homeless, mentally ill, and/or using illicit substances
5.
Evaluates Veterans' physical and mental health, family and social history to identify health care needs
6.
Orders and interprets diagnostic tests
7.
Prescribes medications in accordance with scope of practice
8.
Maintains expected work load panel of patients, assuring their continuity of care, health maintenance, and meeting performance m! easures are a priority
9.
Completes all relevant clin! ical reminders and participates in performance and quality improvement activities
10.
Uses the VA electronic health record "CPRS" to document all patient visits, evaluations, assessments, care plans; and orders, drugs and devices
11.
Provides health education to Veterans, individuals and groups
12.
Collaborates with team members to provide continuity of clinical care and implements a treatment plan
13.
Participates in patient, staff and community education
14.
Participates in medical and/or nursing research and quality improvement
QUALIFICATIONS REQUIRED:
1.
A MasterÂ's Degree in Nursing (MSN) or Doctorate of Nursing (DNP)
required
2.
A graduate of a school of professional nursing approved by the appropriate State agency
and
accredited by the National League for Nursing Accrediting Commission (NLNAC)
or !
the Commission on Collegiate Nursing Education (CCNE). In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirements of graduation from an approved school of professional nursing; however a Commission of Graduates of Foreign Nursing Schools (CGFNS) documentation will be required.
3.
Current Prescriptive/Furnishing License is
required
4.
Current DEA certificate for controlled substances schedules II-V is
required
5.
National Certification as a Nurse Practitioner (ie. ACNP, ANP) is
required
6.
Basic Life Support (BLS) certification from the American Heart Association, American Red Cross- CPR/AED for Healthcare Providers, or Military Training Network is
required
7.
Advance Cardiac Life Support (ACLS) certification from the American Heart Association! or Military Training Network is
required
8.
! Full, active, current and unrestricted registration as a Registered Nurse Practitioner in a State, Territory, or Commonwealth of the U.S. or in the District of Columbia.
9.
Computer literacy proficiency with in-dept experience with the use of Microsoft Office programs or comparable word processing, spreadsheet and graphic software and the ability to learn new programs specific to the VA.
10.
Minimum (3) three years of previous NP experience in an Ambulatory or Primary Care setting
11.
Knowledge and experience with comprehensive Diabetes Management
12.
Must have a valid Driver's License as this position requires driving and seeing Veterans in the community
13.
Experience working with veterans and/or homeless, mental health and substance abuse populations preferred

PHYSICAL REQUIREMENTS:
This position requires visual acuity, keen hearing, clear distinctive speech, and manual dexterity. This position requires potentially long periods of continued walking, standing, stooping, sitting, bending, kneeling, pulling, and pushing. Transferring patients and objects may be required. The candidate may be exposed to infected patients and contaminated materials and may be required to don protective clothing in isolation situations or operative/invasive procedures. The candidate may occasionally be exposed to patients who are combative secondary to delirium, dementia, or psychiatric disorders. The incumbent must be a mature, flexible, sensible individual capable of working effectively in stressful situations, able to shift priorities based on patient needs. The candidate will be working closely with others and may have protra! cted or irregular hours of work.
Applicant's education and len! gth of nursing practice (experience) will be considered by a Nurse Professional Standards Board in determining the grade and salary of the applicant selected. Nurse I Level I - An Associate Degree (ADN) or Diploma in Nursing, no experience.

Nurse I Level II - An ADN or Diploma in Nursing and approximately 1 year of experience, or an ADN or Diploma in Nursing and a bachelor's degree in a related field and no experience; or a Bachelors of Science in Nursing (BSN) and no experience.
·
Nurse I Level III - An ADN or Diploma in Nursing and approximately 2-3 years of experience, or an ADN or Diploma in Nursing and a bachelors degree in a related field and approximately 1-2 years of experience; or a BSN with approximately 1-2 years of experience, or a Master's degree in nursing (MSN) or related field with a BSN and no experience.
