Job Description: Reporting to the Director of Care
Management, this position is the catalyst in managing
care for desired patient outcomes. The care manager
manages the delivery of care throughout the patients
entire episode of illness and evaluates the delivery of
care in terms of patient outcomes and satisfaction. The
care manager collaborates with other hospital
departments and members of the interdisciplinary team to
coordinate all aspects of care and

Job Requ! irements:
Requirements

• AS or BS degree in nursing from an accredited school

of nursing preferred.

• Current California State License as a registered nurse

or LVN required.

• Five years of experience in acute care nursing

preferred.

• One year Care management or discharge planning

experience is desirable.

• Current CPR certification required.
• Case Management Certification preferred
