Silverado is presently seeking a Geriatric Care Manager to join our At Home Los Angeles team. This is a full time position during the day, evening, on-call, and/or weekend shift. Qualifications:
Education: Master's Level Social Worker (MSW or LCSW) or Registered Nurse (RN, BSN, MSN).
Experiences: Several years of experience in geriatric care management, home care, long-term care or other senior-service related field.
Other: A successful track record indicating a working knowledge of clinical geriatric care issues, family dynamics and interventions, education and training, supervision, quality assurance, customer service and marketing available services. Driver's license, insurance and willingness to drive ! to client locations. Summary of Major Functions:
The Geria! tric Care Manager directs the care plan and interventions designed for specific individuals and families.
