Provide direct and indirect patient care in the medical/surgical setting. Communicates with physicians, nurse manager, c! o-workers as appropriate about changes in patient’s clinical condition including results of diagnostic studies and symptomatology. Is able to respond quickly and accurately to changes in condition or response to treatment. Additionally, is able to perform general nursing duties in all departments with adequate supervision.

Essential Functions:

  • Ability to perform an individualized patient assessment, prioritizing the data collection based on the adult or elderly patient’s
immediate condition or needs within timeframe specified by corporate policies.

  • Ability to adequately assess and reassess pain. Utilize appropriate pain management techniques. Educate the patient and family
regarding pain management.

  • Ability to handle a CODE BLUE using BLS protocol.
  • Ability to revise plan of care as indicated by the patient’s response to treatment and evaluate overall plan daily for effectiv! eness.
  • Perform patient care responsibilities consider! ing needs specific to the standards of care for patient’s age.
  • Systematically evaluates the quality and effectives of nursing practice.
  • Acquires and maintains current knowledge in nursing practice.
  • Contributes to the professional development of peers, colleagues, and others.
  • Decisions and actions on behalf of patients are determined in an ethical manner.
  • Knowledge of medications and their correct administration based on age of the patient and their clinical condition.
  • Follow the five medication rights and reduces the potential for medication errors.
  • Formulates a teaching plan based upon identified learning needs and evaluates effectiveness of learning. Family is included and
teaching is appropriate.

  • Demonstrate an ability to assist physicians with procedures and performs services requiring technical and manual skills.
  • Demonstrate ability to perform treatments and provide s! ervice to level of licensure.
  • Treats patient and their families with respect and dignity. Identifies and addresses psychosocial, cultural, ethnic, and
religious/spiritual needs of the patients and their families. Functions as a liaison between administration, patients, physicians and

other healthcare providers.

  • Interacts professionally with patient/family and involves patient/family in the formation of the plan of care.
  • Considers factors relating to safety, effectiveness, and cost in planning and delivering patient care.
  • Demonstrates knowledge of the principles of growth and development of adult patient and the skills necessary to provide age
appropriate care to the patient population served. Able to interpret data about the patient’s status in order to identify each patient’s

age specific needs and provide care needed by the patient group.

  • Performs all aspect of patie! nt care in an environment that optimizes patient safety and reduces the! likelihood of medical/health care
errors.

  • Consults other departments as appropriate to provide for an interdisciplinary approach to the patient’s needs.
  • Maintains a good working relationship both within the department and with other departments.
  • Supports and promotes excellence in customer/guest relations.
  • Communicates appropriately and clearly to Nurse Manager, co-workers, and physicians.
  • Identifies and address psychosocial needs of patients and family, communicates with Social Services/Discharge Planner regarding
both in hospital and post discharge needs.

  • Documentation meets current standards and policies.
  • Manages and operates equipment safely and correctly.
  • Demonstrates an ability to be flexible, organize and function under stressful situations.
  • Demonstrates maturity and accountability for job performance. Supports the philosophy, objectives and goa! ls of Fresno Heart &
Surgical Hospital and assesses of personal and professional growth.

  • Adheres to FHSH Quality and Service Standards of Excellence.
Additional Abilities:
Coordinates and supervises patient care as necessary.

Able to communicate effectively in English, both verbally and in writing.

Basic computer knowledge.

REQUIRED QUALIFICATIONS

EDUCATION:
  • Graduate of an accredited school of nursing.
EXPERIENCE:
  • One year acute hospital experience preferred.
LICENSES AND CERTIFICATIONS:
  • Current CA Registered Nurse Licensure.
  • Current BLS certification.
  • ACLS encouraged
  • CBN or Medical Surgical Certification (RN-C) encouraged

Register! ed Nurse Career Ladder

Four levels of clinical expertise will ! be recognized. Each level builds on the previous level and differentiates nursing practice based on clinical proficiency, experience and the nurse's ability to set and achieve clinical and professional goals. The program addresses the novice to expert nurse as documented in the literature, with specific criteria for each level:

Clinical Nurse (CN) I

. Novice-to-Advanced Beginner Nurse. This nurse is a new graduate RN who:

Completes the state-licensing exam successfully.

Successfully participates in and completes the RN Residency Program, demonstrating acceptable performance as an RN Resident.

Is moving toward a validated status for all core competencies.

Complies with policies and procedures by following established guidelines and using all available resources.

Actively seeks knowledge for professional growth and development.

Participates in mandatory inservices.

Attends 50% of department meeti! ngs, reads minutes of meetings missed within 1 month of the meeting.

OR

an RN with 6 months to 2 years experience, or an RN returning to practice after being out of acute care nursing for five or more years who:

Completes an individually designed orientation program, demonstrating acceptable practice as an RN.

Is moving toward a validated status for all core competencies.

Complies with policies and procedures by following established guidelines and using all available resources.

Actively seeks knowledge for professional growth and development.

Participates in mandatory inservices.

Attends 50% of department meetings, reads minutes of meetings missed within 1 month of the meeting.

CN II - Competent Nurse

. This nurse is a competent clinician who provides nursing care in a skilled and effective manner, utilizing appropriate technical and decision making skills. Promotion to CN II occurs w! hen the nurse achieves a score of 35 on the RN Career Ladder Competency! Model and meets the following criteria:

Meets base requirements of CN I.

Has minimum 2 years hospital experience or is able to demonstrate required competence.

Has achieved a validated status for all core competencies.

Complies with policies and procedures by following established guidelines and using all available resources.

Meets annual requirements for CMC organization-wide competencies.

Maintains certifications required by department (ACLS, NRP, etc.).

Assists with orienting new employees and/or students. May be assigned as Preceptor or Charge Nurse.

CN III - Proficient Nurse

. This nurse is a proficient clinician who provides nursing efficiently and effectively using a high level of technical and organizational skills, critical thinking ability, as well as demonstrating leadership on their unit. Promotion to CN III is accomplished through application to the Nurse Manager when the RN achie! ves a score of 50 on the RN Career Ladder Competency Model and meets the following criteria:

Meets all CN II requirements.

Applies and interviews for the position of CN III.

Has minimum 5 years hospital experience or is able to demonstrate required competence.

Maintains a validated status for all core competencies.

Completes annual requirements for CMC organization-wide competencies on time.

Maintains certifications required by department (ACLS, NRP, etc.), renewals have been completed on time.

Attends 75% of department meetings, reads minutes
